A 35 yr male patient came to the hospital with the complaint of pedal oedema ,Sob and decreased urine output from past 3 days.

History of present illness:
He was the known case of CKD on MHD.Patient was apparently asymptomatic 3 years ago.Then he developed pain in loin region for which he went to private hospital in Mumbai.He was kept on first dialysis on  3rd of November 2021


Past history:
He was K/C/O hypertension since 3 years on Nicardia of 20 mg /Bd and diabetic since 2 years on insulin on   Atropid / Tid.

Personal history:

no loss of appetite

 Diet- mixed

Bowel movements-Regular 

Addictions-nil

Family history:

 No h/o similar complaints in the family

General examination:

Patient is C/C/C.
